Vzájemné působení mezi zdravím ústní dutiny a gastrointestinálními poruchami: komplexní přehled nových souvislostí a klinických implikací

Súhrn

Nové důkazy naznačují obousměrný vztah mezi zdravím ústní dutiny a gastrointestinálními (GI) poruchami, avšak tato souvislost zůstává v klinické praxi nedostatečně prozkoumána. Tato komplexní narativní studie zkoumá současné důkazy spojující dysbiózu ústního mikrobiomu, parodontální onemocnění a další parametry zdraví ústní dutiny s různými GI poruchami. Naše studie identifikovala významné souvislosti mezi parodontálním onemocněním a zvýšeným rizikem idopatických střevních zánětů (IBD) a kolorektálního karcinomu. Dysbióza ústní mikroflóry, charakterizovaná zvýšeným výskytem patogenních bakterií, včetně Porphyromonas gingivalis a Fusobacterium nucleatum, byla spojena se zhoršením příznaků IBD a zánětem střev. Navíc gastroezofageální refluxní choroba (GERD) vykazovala významnou korelaci s erozí zubů, parodontitidou a lézemi ústní sliznice. Mechanistické studie naznačují, že základem těchto souvislostí může být translokace bakterií z ústní dutiny do střeva, systémové zánětlivé mediátory a společná imunitní dysregulace. Závěry: Tato studie zdůrazňuje významnou vzájemnou souvislost mezi zdravím ústní dutiny a gastrointestinálními poruchami a klade důraz na potenciál integrovaných klinických přístupů. Doporučené klinické implikace zahrnují komplexní hodnocení zdraví ústní dutiny u pacientů s gastrointestinálními poruchami, parodontální terapii jako doplňkovou léčebnou strategii a interdisciplinární spolupráci mezi gastroenterology a zubními lékaři. K určení příčinných souvislostí a vypracování integrovaných léčebných protokolů založených na důkazech jsou zapotřebí další prospektivní longitudinální studie a klinické zkoušky.
