Digestive Endoscopy in the Diagnosis of Clinical Entities Manifested in Children by Giant Gas-tric Rugae
Význam digestivní endoskopie v diagnostice klinických jednotekprezentujících se u dětí obrovitými žaludečními řasami
Autoři uvádějí svoje zkušenosti s diagnostikou chorob zažívacího traktu provázených obrovitými žaludečnímiřasami u tří dětí. V prvním případě se jednalo o Ménétrierovu chorobu s foveolární hyperplazií, ve druhémpřípadě o Zollingerův-Ellisonův syndrom s glandulární hyperplazií podmíněný multifokálním gastrinomem v ját-rech a ve třetím případě o gastritidu s obrovitými řasami, histologicky pak s nespecifickými změnami. U třetíhopacienta byl též nález vyklenutí do lumen žaludku způsobený tlakem sleziny do oblasti velké kurvatury.V diagnostice obrovitých žaludečních řas je důležitá digestivní endoskopie a histologie žaludeční sliznicemetodou hluboké biopsie (makrobiopsie). Doplňující laboratorní a zobrazovací metody jsou nezbytné k přesnédiagnostice chorob s nálezem obrovitých žaludečních řas. Vztah infekce Helicobacter pylori k těmto onemocněnímprovázenými obrovitými žaludečními řasami v dětském věku není zatím zcela jasný.

The authors present their experience with the diagnosis of digestive diseases associated with giant gastric rugaein three children. In the first case Ménétriere’s disease with foveolar hyperplasia was involved, in the second caseZollinger-Ellison’s syndrome with glandular hyperplasia caused by a multifocal gastrinoma in the liver and in thethird case gastritis with giant rugae and non-specific histological changes. In the third patient there was also bulginginto the gastric lumen caused by the pressure of the spleen into the area of the greater curvature.In the diagnosis of giant gastric rugae digestive endoscopy and histology of the gastric mucosa by deep biopsy(macrobiopsy) is important. Supplementary laboratory and imaging methods are essential for accurate diagnosisof diseases associated with the finding of giant gastric rugae. The relationship of Helicobacter pylori infection andthese diseases associated with giant gastric rugae in child age is not clear so far.
