Diabetic Microvascular Complications in Patients with Chronic Pancreatitis
Diabetické mikrovaskulárníkomplikace u nemocnýchs chronickou pankreatitidou
U 23 mužů s chronickou pankreatitidou a diabetem, z nich u 19 s etylickou etiologií, bylavyšetřena sekrece C-peptidu po stimulaci glukózou a zjišťována prevalence diabetické nefropa-tie, retinopatie a polyneuropatie. U 12 nemocných byla potřebná ke kompenzaci diabetu léčbainzulinem, u 11 perorálními antidiabetiky. V celém souboru byla prevalence incipientní nefro-patie 21,8 % a neproliferativní retinopatie 8,7 %. Polyneuropatie se vyskytovala pouze u ne-mocných s etylickou pankreatitidou a činila 63,1 %. Při zachované reziduální sekreci inzulinuse u nemocných s chronickou pankreatitidou a diabetem vyskytují nefropatie a retinopatiev incipientních stadiích. Na neuropatii se nepochybně podílí efekt alkoholu. Je možné, že přidůsledných opatřeních zabraňujících progresi chronické pankreatitidy zůstane diab etes a jehokomplikace stacionární.

In 23 men with chronic pancreatitis and diabetes, incl. 19 with an ethylic etiology, the secretionof C-peptide was assessed after stimulation with glucose and the prevalence of diabeticnephropathy, retinopathy and polyneuropathy was assessed. In 12 patients for compensationof diabetes insulin treatment was necessary, in 11 oral antidiabetics were indicated. In thewhole group the prevalence of incipient nephropathy was 21.8% and of non-proliferativeretinopathy 8.7%. Polyneuropathy was found only in patients with ethylic pancreatitis andamounted to 63.1%. If residual insulin secretion is preserved in patients with chronic pancre-atitis and diabetes nephropathies and retinopathies are encountered in incipient stages. Inneuropathies beyond doubt the effect of alcohol participates. It is possible that if systematicmeasures are taken to prevent progression of chronic pancreatitis diabetes and its complicationswill remain stationary.
