Evaluation of left ventricular and left atrial deformation parameters in obese patients

Overview

Obesity is a significant and independent risk factor for the development and progression of cardiovascular diseases, including heart failure with preserved ejection fraction (HFpEF). Diagnosis of HFpEF in obese patients is often complicated by the overlapping symptoms of obesity and heart failure. Speckle Tracking analysis (ST) is a modern echocardiographic method that allows the assessment of deformation parameters of the left ventricle (LV) and left atrium (LA), thus providing a more detailed view of subclinical myocardial impairment and evaluating cardiac involvement in the context of frequently nonspecific symptoms. LV global longitudinal strain (LV-GLS) and LA reservoir strain (R-LAS) represent key parameters, which can detect early myocardial dysfunction even in stages when the LV ejection fraction might still be considered normal. Given the increasing prevalence of obesity, early detection of myocardial involvement is crucial for timely and effective therapeutic intervention of the condition itself as well as associated cardiovascular comorbidities.
