Hepatorenální selhání po sectio caesarea - kazuistika
HELLP syndrom je jedna z forem pozdní gestózy, která je charakterizována hemolýzou (H), zvýšenými jaterními enzymy (EL - elevated liver enzymes)a poklesem počtu trombocytù (LP - low plateled count). Jedná se o vážnou komplikaci těhotenství, spojenou se zvýšenou mortalitou. Vyskytuje sev 70 % před porodem, ve 30 % po porodu, zpravidla od 17. týdne těhotenství do 6 dnů po porodu. Mateřská mortalita je až 50 %. Patofyziologie nenípřesně známa, pravděpodobně jde o důsledek poškození cévní stěny s mnohočetným postižením orgánů(především jater) a aktivace koagulace.K syndromu řadí většina autorů i případy bez hemolýzy (ELLP sy) nebo bez poklesu trombocytů (HEL sy).

HELLP syndrom is one of the forms of late gestosis. It is characterized by H (hemolysis), EL (elevated liver enzymes) and LP (low plateled count).This syndrom is a severe complication of pregnancy with high mortality. Maternal mortality amounts to 50 %. HELLP syndrom occurs as a rule fromthe 17th week of the pregnancy to the 6th day of the postpartum. (70 % before delivery, 30 % postpartum). Pathophysiology is not known exactly, perhapsit is caused by vascular lesion with multiple organ failure (above all of the liver) and by activation of coagulation. What most authors also consideras this syndrom are the cases without hemolysis (ELLP) or without decrease of trombocytes (HEL).
