Chirurgické řešení objemných skrotálních kýl

Z široké škály možností chirurgického řešení tříselných kýl se v případě velkých skrotálních kýl nejčastěji používá plastika síťkou podle Lichtensteina či Prolen Hernia System (PHS). Kontraindikací operace je pouze generalizované maligní onemocnění a špatný kardiopulmonální stav nemocného. Rizika operace jsou především pooperační hematom, infekce síťky, zhoršení dechových funkcí popř. i srdečního žilního návratu, atrofie varlete. Prezentujeme kazuistiku 48letého muže s oboustrannou objemnou skrotální kýlou, operovanou ve dvou dobách podle Lichtensteina.

Overview

The scope of options for the inguinal hernias management is wide. However, in case of major scrotal hernias, the Lichtenstein hernioplasty, using a mesh, or the Prolen Hernia System (PHS), are the most commonly used methods. Only in cases of generalized malignant disorders and cardiopulmonary insufficiencies, the procedure is contraindicated. Postoperative haematoma, mesh infection, deterioration of the pulmonary functions, resp. deterioration of the cardiac venous return and testicular atrophy are the risks of the procedure. The authors present a case-review of a 48-year old male with bilateral scrotal hernias, operated in two stages according to Lichtenstein.
