The Use of Nitric Oxide in the Treatment of Acute Respiratory Failure

Východisko.
Koncem 80. let minulého století byl oxid dusnatý použit terapeuticky jako významný selektivní vazodilatátor v léčbě akutního respiračního selhání provázeného vysokou plicní cévní rezistencí u dětí i dospělých.
Metody a výsledky.
Práce shrnuje fyziologické aspekty působení oxidu dusnatého v organizmu. Zároveň je předložen soubor 33 pacientů (skupina I – 26 novorozenců, skupina II – 7 dětí), u kterých byla v léčbě použita selektivní plicní vazodilatace pomocí oxidu dusnatého. Podle reakce na oxid dusnatý byli rozděleni na podskupinu respondér a nonrespondér. K hodnocení stavu oxygenace byly použity indexy OI (oxygenační index) A-aDO₂ (alveoloarteriální diference) a paO₂ /FiO₂.
Výsledky.
Ve skupině I (novorozenci) byly zjištěny statisticky významné rozdíly uvedených indexů mezi respondéry a nonrespondéry. Ve skupině II se statistická významnost nepodařila prokázat. Výsledky lze srovnat s literárními údaji.

Overview
Background.
Nitric oxide was used as an important selective vasodilator in the treatment of acute respiratory failure accompanied with high pulmonary resistance in children and adults since late 80’s.
Methods and Results.
Paper includes remarks about nitric oxide physiology in organism. Group of 33 patients is presented (group I 26 newborns, group II 7 children) in which selective pulmonary vasodilation with nitric oxide was used. According to response to NO subject were classified into subgroups of responders, non-responders. In evaluation of oxygenation status OI (oxygenation index), A-a DO₂ (alveoloarterial difference) and paO₂/FiO₂ were used.
Conclusions.
Significant differences of above mentioned values were revealed between responders and non-responders in group I (newborns). Significant differences were not revealed in group II (children). Results are in accordancewith other papers.
