Our First Experience with Protocol Biopsy in Transplanted Kidney

Cílem protokolární biopsie štěpu po transplantaci ledviny bylo zjistit výskyt chronické transplantačnínefropatie (CTN) a jejích vztahů ke klinickým a laboratorním ukazatelům.Metody a výsledky. Protokolární biopsie byla provedena u 105 nemocných se stabilizovanou funkcí štěpu 1 rok potransplantaci ledviny. CTN byla nalezena v 75 %, v 6 % byla zjištěna akutní subklinická rejekce. Byl potvrzenstatisticky významný vztah CTN k věku příjemce, k rejekcím prodělaným v 1. roce po transplantaci, k sérovémukreatininu a clearence kreatininu a k proteinurii. Nebyl prokázán rozdíl ve výskytu a rozložení stupňů CTNmezi pacientyléčenými imunosupresí s cyklosporinem a s takrolimem. Za 12 měsíců po biopsii nebyl zjištěn žádný významný rozdílve funkci transplantované ledviny. U nemocných s takrolimem byly významně nižší hodnoty cholesterolu a triglyceridů,jejich další pokles v průběhu 12 měsíců však byl podobný poklesu u léčených cyklosporinem.Závěry. CTN byla nalezena ve většině protokolárních biopsií provedených v 1. roce po transplantaci ledviny,v ojedinělých případech pak odhalila subklinickou akutní rejekci.

Background.
The aim of protocol biopsy after renal transplantation was to assess the prevalence of chronic allograftnephropathy (CTN) and to correlate the degree of CTN with clinical and laboratory data.Methods and Results. In 105 patients with a stabilized graft function, a protocol biopsy was carried out at 1 yearafter transplantation. CAN was found in 75% of patients, and in 6% an acute subclinical rejection was revealed.Statistically significant correlation was confirmed between CAN and recipient’s age, development of acute rejectionin the first year posttransplant, serum creatinine, clearance of creatinine, and proteinuria. There was no significantdifference in CAN degree distribution between patients treated with cyclosporine-A or with tacrolimus. Twelvemonths after the biopsy, there was no significant change in kidney graft function. In patients treated with tacrolimus,cholesterol and triglycerides levels were significantly lower than in cyclosporine treated patients Over the next year,these values significantly decreased in both subgroups.Conclusions. The CAN was found in the majority of protocol biopsies at 1 year after kidney transplantation;subclinical acute rejection was revealed rarely.
