The role of the pharmacist in the management of anticoagulant therapy with warfarin –⁠ literature review

Introduction and aim: Warfarin treatment is associated with the risk of bleeding and thrombotic events. Although it is used less frequently, it still holds an important position in a specific patient population. Therefore, it is necessary to find ways to improve the management of its administration. One option is the implementation of a pharmacist in the anticoagulation team. The aim of the work is to provide a literature review of the role and contribution of the pharmacist in the anticoagulation management of warfarin and the possibilities of pharmacist implementation in the anticoagulation team.

Methods: When searching for suitable works, the PubMed bibliographic database was used with the key words „warfarin“ and „pharmacists“ in the period January 2015 to May 2025. Meta-analyses, reviews, systematic reviews, and works that were not written in English were excluded.

Results: Based on the defined parameters, 30 works were selected that were exclusively devoted to the role of the pharmacist and related contribution in anticoagulation therapy with warfarin. Most publications focused on the influence of the pharmacist on achieving the required time in the therapeutic range, to a lesser extent on adverse effects, mortality, patient adherence to warfarin treatment, costs, patient satisfaction and quality of life, improving their knowledge.

Conclusion: The pharmacist has an important position in the team caring for patients taking warfarin. Pharmacist implementation brings about an improvement in the quality of anticoagulation management and minimization of risks.

Overview

Úvod a cíl: Léčba warfarinem je spojena s rizikem krvácení a trombotických příhod. Přestože je používán čím dál méně, stále si zachovává významné postavení u specifické populace pacientů. Proto je třeba hledat způsoby, jak zkvalitnit management jeho podávání. Jednou z možností je implementace farmaceuta do antikoagulačního týmu. Cílem práce je poskytnout literární přehled o roli a přínosu farmaceuta v antikoagulačním managementu warfarinu a o možnostech jeho implementace do antikoagulačního týmu.

Metodika: Při vyhledávání vhodných prací byla použita bibliografická databáze PubMed s klíčovými slovy „warfarin“ a „pharmacists“ v časovém období leden 2015 až květen 2025. Byly vyřazeny metaanalýzy, přehledové práce, systematické přehledové práce a práce, které nebyly psány v angličtině.

Výsledky: Na základě definovaných parametrů bylo získáno 30 prací, které se věnovaly výhradně roli farmaceuta a jeho přínosu v antikoagulační terapii warfarinem. Nejvíce publikací bylo zaměřeno na vliv farmaceuta na dosažení potřebného času v terapeutickém rozmezí, v menší míře potom na nežádoucí účinky, mortalitu, adherenci pacientů k léčbě warfarinem, náklady, spokojenost a kvalitu života pacientů, zlepšení jejich znalostí.

Závěr: Farmaceut má významné postavení v týmu starajícího se o pacienty užívající warfarin. Jeho implementace přináší zlepšení kvality antikoagulačního managementu a minimalizaci rizik.
