Prophylaxis of Venous Thromboembolic Disease in Patients after the Proximal Femoral Surgery
Profylaxe žilní trombózy po operaci proximálního femuru u starých osob
Pacienti, kteří se podrobí operaci kyčelního kloubu, ať již pro zlomeninu, nebo elektivně, patří do skupiny nemocných s nejvyšším rizikem vzniku tromboembolické nemoci, což je do značné míry vylučitelná příčina morbidity a mortality. Ačkoliv vztah mezi těmito operacemi a rizikem vzniku žilního tromboembolizmu je znám již mnoho let, je v profylaxi žilního tromboembolizmu v klinické praxi stále mnoho nejasností. Cílem tohoto článku je ozřejmit výskyt a rizikové faktory hluboké žilní trombózy a plicní embolie po operacích proximálního femuru a poskytnout na důkazech založená doporučení vycházející ze 7. konference Americké akademie plicních lékařů.

Overview
Patients undergoing elective hip replacement and surgery for hip fracture are at increased risk of venous thromboembolic disease, a potentially preventable cause of mortality and morbidity. Although the association between these operations and venous thromboembolism has been recognized many years ago, there is still great variability in the clinical practices with respect to prophylaxis. This article aims to clarify the incidence and risk factors for deep venous thrombosis and pulmonary embolism after hip operations, and give evidence-based recommendations for prophylaxis adopted from the Seventh American College of Chest Physicians Conference (ACCP).
