Výskyt Diamondovy-Blackfanovy anémie v České republice

Ve spolupráci s dětskými hematologickými centry v České republice byl vytvořen registr pacientů s Diamon-dovou-Blackfanovou anémií (DBA). Bylo vyšetřeno 26 pacientů s chronickou hypoplastickou anémií. U 22 z nichbyla stanovena diagnóza DBA, což odpovídá incidenci 7 případů na 1 milion živě narozených. U zbývajících 4pacientů byla u dvou dětí diagnostikována chronická infekce parvovirem B19, u jednoho Fanconiho anémiea u jednoho atypicky probíhající tranzientní erytroblastopenie. Soubor pacientů s DBA tvoří 11 chlapců a 11děvčat ve věku 4 měsíce -⁠ 29 let. Byly nalezeny 3 rodiny s výskytem více případů DBA, ve 2 rodinách se uplatňujeautosomálně dominantní typ dědičnosti, ve 3. rodině autosomálně recesivní typ. Diagnóza DBA byla u všechpacientů souboru stanovena do 10 měsíců věku. Anémie byla zjištěna u 11 (50 %) pacientů již v novorozeneckémvěku. Čtyři (18 %) pacienti se narodili s porodní hmotností pod 5. percentilem pro gestační věk, čtyři (18 %) dětise narodily předčasně (35.-37. týden gravidity). Overview

In cooperation with paediatric haematologists from the Czech Republic, the Diamond-Blackfan anaemia (DBA)registry was created. Twenty six patients with chronic hypoplastic anaemia were investigated. Twenty two patientsmet the diagnostic criteria for DBA. The incidence of DBA in our country estimated from our registry is 7 DBA cases per million live births. The other 4 patients suffered from chronic parvovirus B 19 infection (2), Fanconianaemia (1), transient erythroblastopenia (1). In the group of DBA patients are 11 boys and 11 girls, the age rangeis 4 months to 20 years. There are 3 families with more than one case of DBA in the registry, two families withautosomal dominant and one with an autosomal recessive inheritance. Diagnosis of DBA was established duringfirst 10 months of life in all cases. Eleven patients (50%) were anaemic already in neonatal age.
